Regulatory & Licence Breach Investigation — Pakistan & International

The notice has arrived. What you do next matters most.

An inspection finding, a licence condition breached, a permit exceeded, a certification audit failed. We establish the facts independently, find why the control failed and whether it has failed elsewhere, and build the corrective plan your response to the regulator rests on.

Six stages. What is expected, and what we provide.

First response · what is usually expected

Prompt acknowledgement and immediate steps to control any risk.

What P3i provides

We help identify the interim controls that should be in place this week.

A finding tells you what was wrong on the day. It does not tell you why, or where else.

Regulators in many countries take account of how quickly and credibly an organisation investigates a breach and puts it right. A corrective plan built on a real investigation is more convincing, and more likely to work, than one written to meet a deadline.

What we are asked to look at

Inspector walking a factory floor with a clipboard
Inspection findingsSafety, fire, labour or environmental findings from a site inspection.
Framed operating licence on an office wall
Licence and permit conditionsConditions breached, limits exceeded, or obligations not met on time.
Certification audit in progress at a desk
Certification auditsMajor non-conformities that put a certificate at risk.
Buyer's social compliance auditor on a site visit
Buyer and customer auditsCritical findings in a brand's social, safety or environmental audit.
Internal audit report binders on a shelf
Repeat internal findingsThe same finding in every internal audit, closed each time and back each time.
Compliance register open on a laptop
Missed obligationsRenewals, tests and reports that lapsed because nobody owned them.

A notice with a deadline? A first call costs nothing, and we will tell you what should be in place before your response is due.

Talk to an investigator WhatsApp

You commission one investigation. You get a review of the whole system.

One finding. Compliance that holds.

Register

Every obligation listed, with an owner and a due date.

Evidence

Records kept so compliance can be shown, not just claimed.

Checks

Internal checks timed before external inspections, not after.

Closure

Findings closed only when the cause is fixed, not when the paperwork is.

Corrective action plan pinned on a board

Examples of what we typically recommend after a regulatory finding.

The P3i approach

How we investigate a regulatory finding. Fact-finding, not fault-finding.

Every P3i investigation is led by a senior investigator and supported by internationally qualified process specialists. You will not receive a list of people to blame, or a long list of generic actions. You will receive the facts, the proven causes, and a short plan that closes the gaps, in this case and across your system.

Independent

No products, repairs or contracts to sell you afterwards.

Senior-led

Thirty years of investigation, from the first call to the final briefing.

Beyond the event

Process specialists find the other cracks before they fail.

Ready to present

Written for boards, buyers, insurers and regulators.

Free 30-minute case assessment.

Tell us what happened. In confidence, we will tell you what the investigation should cover, how long it will take and what to preserve today.

How we work with you

What happens after you call.

For a regulatory finding, the first call covers the deadline, the interim controls that should be in place now, and what records the response will need.

  1. First hour

A call with a lead investigator. You leave it knowing what to preserve today and who to call next.

  2. Day one

An investigator on site, anywhere in Pakistan. Scope and fixed fee agreed in writing.

  3. Week one

A private briefing on early findings, so urgent risks are dealt with before the report is finished.

  4. Weeks three to four

The final report, the wider system review and a remedial work plan, presented to your team.

  5. After

We come back to check the actions were completed and actually worked.

Typical timings for a single-site event. Complex, multi-party matters take longer, and we tell you so on the first call.

What you receive

A full investigation report. This is only its first page.

The summary is what leadership reads first. Behind it sits the complete file: every witness heard, every document and record examined, and every finding tied to numbered evidence, so a board, an insurer, a buyer or a regulator can test it.

Questions we are asked

By compliance and legal heads.

Do you represent us before the regulator?

No. Legal representation stays with your lawyers, and the relationship with the regulator stays with you. We provide the independent facts and the plan your response is built on.

We know what the finding was. Why investigate?

The finding shows what was wrong on the day. An investigation explains why, and whether the same gap exists elsewhere, which is what a credible plan must address.

Do you help with certification and buyer audits?

Yes. The same approach works for certification, buyer and internal audit findings.

Will you only look at this finding?

No. Our process specialists review how obligations are tracked, owned and checked, and give targeted actions for the gaps that would produce the next finding.

Respond with facts, not just a deadline.
